If you require any vaccinations relating to foreign travel you will need to complete a travel questionnaire, which includes information about which countries and areas within countries you are visiting to determine what vaccinations are required.  Once the surgery receives the completed questionnaire the practice nurse can review your information, and the surgery will contact you to make an appointment with the practice nurse.

It is important to complete and return the form as early as possible - at least 6 weeks before you travel - to allow time for the practice nurse to review your information, travel vaccines to be ordered and for you to receive the vaccinations at least 2 weeks before you travel. 

Please note that not all travel vaccinations are included in the services provided by the NHS, and are ordered on a private prescription which incur a charge over and above the normal prescription charge.
